Role Description
Be a part of the Clinical Care Team A s a Patient Care Coach you will engage our members to develop lifelong well-being and health.
CAC is seeking a Patient Care Coach to help coordinate and evaluate the management of patients with acute and chronic conditions, as well as patients discharged from acute facilities, to achieve high quality care. You will interact with all levels of nursing and other departments to assure effective utilization of resources that meet the physiological and safety needs of the patient and their families. You will be collaborating with planning, directing and evaluating patients in the Care Coordination System.
Implement physician approved care plans for patients in the Care Coordination System.
Deliver direct patient care, administer prescribed medication, transcribe and implement doctors' orders, and other duties as assigned.
Collaborate with other members of the healthcare team in the planning, directing and evaluating patients in the Care Coordination System.
Monitoring and caring for high risk patients with acute and chronic medical conditions.
Assess patient needs; assist in planning, implementing and evaluating interventions to meet patients and family needs when appropriate.
Record patient care and progress on appropriate documents including patient-specific care plans. Maintain and properly log patient follow up calls as required.
Educate and teach patients and families regarding patient disease and procedures that will directly affect the patient.
Flexible to changes in work schedule and location as needed by the organization.
Maintain standards for professional nursing practice and demonstrate the ability to use the nursing process in setting.
Other duties as assigned.
Role Essentials
Active RN License
Active CPR-BLS from the American Heart Association
Minimum of 2 years of prior experience in clinical setting (acute or post-acute)
Proficiency with computers including Microsoft products, Word, Excel, and Outlook
Strong organizational skills and ability to multi-task effectively
Ability to problem solve and take initiative to best meet patient needs
Must be able to interact and communicate effectively with patients
Must be able to relate well with all levels of individuals within the organization
Must be fluent in English and Spanish
Role Desirables
Bachelor s or Associate s Degree in Nursing preferred
Home health and/or case management experience
4 to 6 years of clinical experience with in a hospital preferred
